Once you have begun the application process at Windsor University School of Medicine (WUSOM), you will need to provide and access quite a bit of information to get started. We want you to take every step necessary to ensure your success at our university. The Downloads You Will Need
Knowing exactly what forms you will need can be confusing at the onset of application and enrollment. Use the following checklist as a guide to exploring the forms and brochures every WUSOM student should have:
• WUSOM Brochure – You can start with our brochure. This document comprises an overview of vital information, such as admission requirements, our academic programs, our accreditations, and what to expect from your time in St. Kitts.
• Student Handbook – This handbook contains a vast assortment of information you should know as a WUSOM student, including our policies and procedures, details about our curriculum, and information about student services. You may find yourself referring to this handbook many times during your time at WUSOM.
• Clinical Student Requirement Checklist – Download this checklist to see exactly what documents clinical students are required to have. These will include your immunization records and health screening, as well as a background check.
• Housing Form – This form should supply the information required to apply for housing as a student at WUSOM.
• Clinical Training Manual – This manual supplies a wealth of details about what to expect from the clinical training process.
• Clinical Health Form (History/Physical) – You will complete this required form to impart necessary information about your health history. An examining physician will also fill out part of this form in conjunction with your physical examination.
• Clinical Health Form (Immunization) – This form details your immunization records.
• Transcript Request Form – You will use this form to request any applicable transcripts from previous educational institutions you have attended.
• Elective Affiliation Agreement Form – This form lists the details of your approval to perform a supervised elective rotation.
• Graduation Questionnaire – This questionnaire should be filled out prior to the processing of your USMLE application.
• Student Exit Interview Form – Upon preparing to leave WUSOM after graduation, you will fill out this form to detail relevant information about your future residency, as well as information about your experience at our Caribbean medical school.
• Visa Form – If you come from a location outside of the area, you will need to complete this form to obtain a visa to stay in St. Kitts.
• Credit Card Form – This is your credit card verification form.
